Agreements

Participation in basketball training, athletic performance, strength and conditioning, practices, scrimmages, games, camps, clinics, leagues, evaluations, and related activities involves inherent risks. These risks may include, but are not limited to, falls, collisions, contact with other participants, equipment-related injuries, overexertion, dehydration, illness, and serious bodily injury.

I understand that Great Training activities may take place at indoor or outdoor courts, gyms, fitness facilities, partner locations, rented facilities, schools, parks, or other training environments used by Great Training.

I voluntarily assume all known and unknown risks associated with participation in Great Training activities for myself and/or my child.

I certify that I and/or my child are physically able to participate in these activities and do not have any condition that would make participation unsafe, unless such condition has been disclosed in writing prior to participation.

I understand that Great Training does not provide medical insurance for participants, and I accept full financial responsibility for any medical care, treatment, or emergency services that may be required as a result of injury, illness, or accident.

To the fullest extent permitted by law, I release, waive, and discharge Great Training LLC, its owners, coaches, trainers, staff, contractors, volunteers, facility partners, and affiliates from any and all claims, demands, causes of action, liabilities, damages, costs, or expenses arising out of or related to participation in Great Training activities, except where prohibited by law.

I understand that Great Training reserves the right to remove any participant whose conduct is unsafe, disruptive, or not aligned with program expectations, and that such removal does not create liability on the part of Great Training.

I acknowledge that I have carefully read and understand this Liability Waiver & Assumption of Risk, and I agree to it voluntarily.

By participating in Great Training programs and activities, I grant permission for Great Training to photograph, record, and use images, video, and audio of me and/or my child as described below.

I authorize Great Training LLC and its representatives, employees, contractors, partners, and affiliates to photograph, videotape, and otherwise record my child’s and/or my participation in Great Training activities, including but not limited to practices, games, camps, clinics, training sessions, evaluations, events, and related programming.

I grant Great Training the right to use these photographs, videos, recordings, and related media for lawful business purposes, including but not limited to marketing, advertising, promotion, social media, website content, digital materials, printed materials, presentations, and other branded communications.

I understand that such media may be edited, copied, displayed, published, distributed, or otherwise used by Great Training without further notice.

I understand that no compensation or payment will be provided to me or my child for the use of these materials.

I release and discharge Great Training LLC, its owners, staff, contractors, partners, and affiliates from any claims related to the use of such media, including claims involving likeness, privacy, or publicity rights, to the fullest extent permitted by law.

I understand that this release will remain in effect unless I submit written notice requesting that Great Training stop future use of newly created media. I understand that materials already produced, published, or distributed before such notice may continue to be used.

I acknowledge that I have read and understand this Media Release and agree to it voluntarily.

By providing my contact information to Great Training, I agree to receive communications related to registration and program participation.

I authorize Great Training to contact me by email and text message regarding registration, scheduling, reminders, updates, announcements, billing, program information, and related business communications.

I understand that message and data rates may apply depending on my mobile carrier and plan.

I understand that consent to receive communications helps Great Training provide important updates related to my child’s participation and registration.

I understand that I may opt out of non-essential promotional communications at any time by following the unsubscribe instructions provided in emails or by contacting Great Training directly.

I acknowledge that I have read and understand this Communication Consent and agree to it voluntarily.
